ASSESSMENT AND DIAGNOSIS

At Vitality Unleashed Psychology, our psychologists provide assessment and diagnosis services to clients and third-parties.

These services are typically requested by third-party payers (e.g., WorkCover Qld, or other insurers). An additional fee for these services will be charged, although requests from third-parties generally pay on behalf of the client. Should you require a letter or report at your own request, this charge will be issued and payable by the client. Please note that Medicare does not cover these fees. Clients may require a letter or report from a Psychologist for legal reasons, disability support or pension applications or other means.

It is important that you discuss the situation and your assessment and diagnosis needs with your Clinical Psychologist so they can best determine the appropriate assistance to provide you with. It is important to advise this at the very start of your session.

Vitality Unleashed Psychology will not release any confidential information to third-parties without your prior written consent. Third-parties may also ask you for your written consent prior to requesting the service. Please note that full payment will need to be received before your assessment can be released.

Fees are determined by the type of assessment in which you require and the timeframe to complete.
