01 New Client Enquiry — Psychological Assessment and/or Therapy Enquiry only · new clients +

This form is for new clients enquiring about Psychological Assessment and/or Psychological Therapy services at Vitality Unleashed Psychology.

Psychological Assessment (also called Clinical Assessment)

A process used to better understand a person’s mental health, emotional functioning, behavioural patterns and psychological wellbeing. It is often undertaken for diagnostic clarification — exploring whether a mental health condition may be present, understanding current symptoms, or clarifying how different difficulties interact with one another. Psychological assessment always involves psychological consultation/s and may involve standardised (free or paid) questionnaires or screening tools. It does not involve a full psychometric testing battery or a comprehensive psychometric report, however these services can also be completed on request where clinically indicated by us.

Psychological Assessment is different from Psychometric Testing. Psychometric testing is a more formal, in-depth process involving multiple standardised tests and a comprehensive report, generally used where detailed cognitive, neurodevelopmental, learning or personality assessment is required. Psychological Therapy

Therapy involves working collaboratively with a Psychologist to address emotional, psychological or behavioural concerns. It may focus on understanding patterns, developing coping strategies, improving wellbeing, processing experiences, or supporting personal growth. Therapy can be provided with or without a formal diagnosis — some people seek therapy following diagnostic clarification, while others engage in therapy even when their mental health is generally stable and no diagnosable condition is identified.

What happens next

This form allows us to understand the nature of your request and to determine whether an initial psychological consultation with us is appropriate. Following review of your enquiry, we will contact you via your preferred method to provide further information and advise on next steps.

You do not have to submit an enquiry to book an initial consultation. You can book online at any time, and we will then email you the intake and consent forms we require to confirm your appointment.
02 Psychometric Testing Enquiry Enquiry only · all ages +

This form is for individuals enquiring about psychometric testing services at Vitality Unleashed Psychology.

Psychometric testing is a structured assessment process that may be used to explore areas such as cognitive functioning, learning, attention, neurodevelopmental concerns, personality functioning, or diagnostic clarification. Detailed information about our testing services and how the process works is available on our Psychometric Testing page, and we recommend reviewing that information before submitting an enquiry.

Submission of this form is an enquiry only

It allows us to understand the nature of your request and to determine whether an initial psychological consultation is appropriate. All psychometric testing requests require an initial consultation before any testing is discussed in detail, quoted, or commenced. Psychometric testing does not proceed without this initial assessment and completion of the clinic’s intake and psychometric testing consent forms.

After reviewing your enquiry we will contact you via your preferred method to provide further information or advise on next steps. If appropriate, you may then book an initial testing consultation online or via SMS request. Bookings are only confirmed once the required intake and consent documentation has been completed by the Client, or by the appropriate legal authority where applicable.

Requests are reviewed to ensure they are clinically appropriate and consistent with professional standards.

This form is for new clients who would like to request an initial appointment with one of our psychologists or our dietitian, but who prefer not to book online or need a little extra support with the process.

If you wish to make an enquiry about our services first, please leave this form and complete a service enquiry form instead — this form is for those who have already decided they wish to proceed with booking an initial psychology or dietitian appointment.

04 Request a Form, Letter or Report Billable clinical service · upload required +

This form is for Clients or authorised third parties to request the completion of a form, letter, or psychological report by a Psychologist at Vitality Unleashed Psychology.

Requests may include, but are not limited to, university or education forms (such as exam adjustments, extensions or special consideration), letters supporting academic, occupational or personal circumstances, NDIS or Disability Support Pension documentation, insurance or WorkCover documentation, and psychological reports for non-psychometric purposes.

This form is not for psychometric testing reports, which are completed as part of a separate assessment process.
Important information about requests

Submission of this form does not guarantee that the requested form, letter or report can or will be completed. All requests are clinically reviewed to determine whether it is appropriate for the Psychologist to complete the request, whether sufficient clinical information is available, whether additional appointments or assessments are required, and the estimated time involved. In some circumstances it may not be appropriate for the Psychologist to complete the requested documentation.

Clinical time, appointments and fees

Completion of forms, letters and reports involves clinical time and is a billable service. Depending on the nature of the request this may include time spent reviewing documentation, drafting and finalising the document, and additional psychological consultations required to gather relevant information. This is particularly important where the requester is a new Client, or where further clinical assessment of an existing Client is necessary. Where documents must be reviewed to complete the request, document review fees apply and form part of the total cost.

Legal and medico-legal requests

Requests for reports or letters for legal purposes must be submitted via the Client’s lawyer, with clear written instructions outlining the scope of the request and the specific questions to be addressed. Usually the Client’s lawyer will have this pre-written in a letter addressed to us.

Process after submission

After submission the request will be reviewed to determine whether it can proceed. If appropriate, an estimated fee will be provided. You will then be issued with a Form, Letter or Report Request Consent Form and an invoice (both emailed via our Client Management System), which must be paid in full before work commences. If additional time is required beyond the original estimate, any outstanding balance must be paid in full before the completed document is released.

Documentation required

You must upload all relevant documentation to support your request — such as the required form template (if your request involves its own template, e.g. a University form), instructions or guidelines from the requesting organisation where applicable, and any relevant correspondence or supporting material. Incomplete requests or missing documentation may delay review or result in the request being declined.

Exclusions

This request process does not apply to routine Provider Management/Treatment Plans (e.g. by MAIC or WorkCover), which are completed as part of standard clinical care and are not billed separately.

05 Submit Funding Pre-Approval Third-party funders · upload required +

Use this form to securely submit formal funding pre-approval documentation for psychological services (e.g. initial psychological assessment consultations, psychological therapy, clinical assessment, psychometric testing, psychological reports) and dietetics services (e.g. initial and subsequent consultations, meal plans, reports).

Funding bodies, insurers, case managers or support coordinators submitting approval directly to us

Please ensure the approval documentation clearly outlines the approved services, funding limits, validity period, and any billing conditions.

06 Medicare Rebate Not Received Existing clients · rebate follow-up +

If you have not received the Medicare rebate/s for any of your attended appointments into your or the Claimant’s bank account, please check the following before completing this form:

1Your bank account details registered with Medicare are correct
2You have a current Medicare card
3You have a valid Medicare referral for our services, and the attended consultation date/s are not prior to the date of the referral
4You have not maxed out attended sessions for the calendar year for that referral type across all providers and clinics (maximum 10 for psychology under MHCP; maximum 40 for psychology under EDMP; maximum 5 for psychology under CDMP)
5That you have not received a Medicare statement with the receipt we emailed you following your attended appointment — if you have received this statement you will need to contact Medicare first, as we do not have permission to do this on the Claimant’s behalf

If you are still of the view that you should have received your Medicare rebate and have not, please proceed with the form below. We can then check our client management system to identify why the rebate has not been received, and will email you the outcome within 2 business days.

07 Confirmation of Attendance Existing clients · consent assessed +

This form is for requests relating to confirmation of attendance at appointments held with Vitality Unleashed Psychology.

Confirmation of attendance may be requested by a Client for their own records, or in limited circumstances by a parent or authorised party where appropriate consent and legal authority exist. Attendance information can be provided in different formats depending on the purpose of the request.

Where a Client is requesting their own attendance history for personal use, an exported list of attended appointments may be emailed directly to the Client. This option does not involve the preparation of a letter and does not include clinical commentary.

Requests for confirmation of attendance in a letter addressed to a third party (such as an employer, educational institution, court, insurer, probation or parole service, or other organisation) are treated as formal documentation requests. These must be submitted via the Request a Form, Letter or Report process, as they involve professional judgement, drafting and clinical responsibility.
Where a request is not submitted by the Client

We are required to assess whether the Client is legally and clinically able to consent to the release of their information. This includes consideration of whether a Child Client is assessed as a Mature Minor.

If a Client is deemed to be a Mature Minor, we require the Client themselves to request confirmation of attendance. We do not obtain or chase consent on behalf of parents, guardians or third parties in these circumstances, and we do not release attendance information based solely on a third-party request.

For Child Clients who are not Mature Minors, confirmation of attendance may be requested by a parent or legal guardian, provided there are no Court Orders or Parenting Orders restricting access to the Child Client’s medical or psychological information. Where such orders exist, attendance information may only be released following a request from the parent who holds legal authority, or where appropriate written consent and documentation have been provided.

Requests submitted by third parties without the Client’s direct involvement will not be actioned. In these situations the Client should submit this form themselves. Attendance information will then be emailed directly to the Client, who may choose to share it with a third party at their discretion.
All requests are reviewed to ensure they are appropriate, lawful and consistent with confidentiality and professional obligations. Submission does not guarantee immediate processing. Incomplete requests, requests that fall outside these boundaries, or requests requiring a different process may be declined or redirected.
08 Case Conference Request Fee-for-service · written consent required +

A case conference is a planned, collaborative meeting between the Client’s treating clinician (psychologist and/or dietitian) from our clinic and other relevant external professionals involved in the Client’s care — for example a GP, psychiatrist, dietitian, school staff, support coordinator, insurer, or legal representative.

Case conferences are not crisis interventions, and are not informal phone calls or email discussions.

09 File Release Request Fee applies · consent form required +

Use this form to request the release of clinical records, reports, or relevant information from your file to a third party, such as a GP, specialist, school, workplace, insurer, or other health professional.

Requests for file release must be made by the client, or by a person with the legal authority to act on the client’s behalf. Appropriate written consent and full payment of the invoice is required before any information can be released.

Please note that file releases are subject to professional, ethical and legal obligations, including privacy legislation and clinical judgement regarding what information is appropriate to share. Some requests may require clarification, and processing timeframes may vary depending on the nature and volume of the information requested.

Once submitted, your request will be reviewed and actioned in line with our clinic’s policies. Once authorised, we will email you a File Release Consent Form to complete along with an invoice for payment. The total fee invoiced before the file is released will be determined by the size of the file to be released.

All file releases incur a file release fee for all clients and third parties, and we cannot proceed with actioning file release requests without this form being completed first. We do not accept file release requests sent to us via email.

This secure form allows clients, parents or legal guardians, and authorised third parties to upload documents relating to psychological care at Vitality Unleashed Psychology.

Documents may be submitted for inclusion on file only, or for professional review by your psychologist. Please note that review of documents outside scheduled appointment time involves clinical time and may be billed in accordance with our current fee schedule. Where applicable, you will be advised before review proceeds.

All information submitted via this form is encrypted and handled in accordance with the Australian Privacy Principles (APPs). Uploaded documents are accessed only by authorised clinicians and administrative staff.

Submitting documents through this form helps ensure your information is received securely, clearly identified, and directed appropriately — reducing delays and unnecessary back-and-forth communication.

Third-party submissions and consent

If you are not the Client and do not have the Client’s knowledge and informed consent to provide information relating to them, please do not submit documents or other information via this form or by any other method.

Documents submitted by a third party may be added to the Client’s clinical file and may be disclosed to the Client in accordance with professional, ethical and legal obligations. Documents submitted without the Client’s consent may not be reviewed, relied upon, or acted upon.
