08 Case Conference Request Fee-for-service · written consent required +

A case conference is a planned, collaborative meeting between the Client’s treating clinician (psychologist and/or dietitian) from our clinic and other relevant external professionals involved in the Client’s care — for example a GP, psychiatrist, dietitian, school staff, support coordinator, insurer, or legal representative.

Case conferences are used to
Share relevant clinical information (with consent)
Coordinate treatment and care across providers
Clarify roles, responsibilities and next steps
Support complex, multidisciplinary needs
Case conferences are not crisis interventions, and are not informal phone calls or email discussions.
All case conferences
Require written Client consent
Must be requested and scheduled in your treating clinician’s consultation diary in advance
Are fee-for-service and billed for clinician time at the clinician’s standard consultation rate
Are subject to clinician approval and availability

Submitting this form allows you to request consideration of a case conference involving psychology and/or dietitian services. Approval and scheduling will be confirmed after review.

If your request does not involve coordinated care with other professionals, please select a different request form or review our FAQs.
