Documents may be submitted for inclusion on file only, or for professional review by your psychologist. Please note that review of documents outside scheduled appointment time involves clinical time and may be billed in accordance with our current fee schedule. Where applicable, you will be advised before review proceeds.
All information submitted via this form is encrypted and handled in accordance with the Australian Privacy Principles (APPs). Uploaded documents are accessed only by authorised clinicians and administrative staff.
